What is a nursing home care plan?

 

A: When you or a loved one is admitted to a nursing home or assisted living facility in Virginia, the facility will evaluate your medical and physical needs. This evaluation is used to create a care plan that instructs the facility’s staff on how you are to be cared for. The care plan may include the types of medications that you require, physical therapy treatments and other activities that will contribute to your overall quality of life. If a nursing home fails to follow a resident’s care plan, they may be held liable.
